About the role
We are looking for an exceptional individual to join our Dementia Support Team. You will provide support and guidance to people with dementia and their carers across Bath and North East Somerset, helping to maintain their independence, improving their sense of well-being, and putting them more in control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs.
The support is provided in a range of settings, including the client’s home, the community and in groups. The support will include enabling service users to have a greater understanding of dementia, plan for the future, as well as signposting and providing referrals to other services. You will liaise with relevant statutory and voluntary organisations in the area to ensure service users receive up to date information.
Your role will require you to deliver a carer’s group within the city of Bath and also provide cover for other groups as and when necessary. The role also requires rostered attendance at a memory clinic surgery where you will provide support for people who have received a diagnosis of dementia. As the role covers a large geographical area, the ability to travel independently would be preferable.
We are looking for an exceptional individual to join our dementia support team. You will need to possess an NVQ Level 3 (or equivalent) and have a good understanding of dementia and the needs of those living with dementia and their carers. An ability to assess the service users’ needs along with an understanding of the importance of confidentiality and data protection is essential.
To be successful in this role you will have excellent communication skills and an understanding of a person centred approach. You should have relevant experience of record keeping and report writing. The role involves regular use of IT packages; therefore a good standard of IT would be desirable.
You will be responsible for your own caseload therefore the ability to manage your own time effectively is essential. Your caseload will be varied so we are looking for someone with the right mix of enthusiasm, resilience, professionalism and flexibility who is able and willing to work in a variety of settings.
Experience of working with volunteers is desirable. The post will involve working in people’s homes and in a range of community settings; therefore a DBS disclosure will be required.
