Alzheimer’s Society is the UK’s leading support and research charity for people with dementia, their families and carers. There are 850,000 people with dementia in the UK and numbers are set to rise to over 1 million by 2025.

The Alzheimer’s Society are working in partnership with Leeds and York Partnership NHS Foundation Trust to deliver a service where Memory Support Workers have the rewarding opportunity to provide support and guidance to connect the person with dementia, and their family/carer, to community support; and integrating primary, secondary and community care in a person-centred approach.  The team will support the management of dementia in primary care and connect directly to initiatives to reduce admissions and maintain personal well-being & independence for as long as possible.

You will possess a  minimum Level 2 Health and Social Care or relevant qualification with an excellent understanding of dementia and the needs of those affected by it. Your ability to provide person centred assessment of need, along with a clear understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.

To be successful in this role you will have excellent non-judgmental communication skills as well as strong organisational and time management skills in order to manage your caseload of clients effectively.

You will work from an Integrated team base and from home across a defined area of Leeds. You will need to access to a wifi connection at home

It is essential you can travel independently across the area.
