Alzheimer's Society is the UK's leading dementia charity. We provide information and support, improve care, fund research, and create lasting change for people affected by dementia.

About the role

We are looking for applicants to provide support to people living with dementia and their carers helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives.

The successful applicant will provide information, signposting and support in people’s homes and at locations across Hillingdon. The post holder will work closely with the memory clinic and the carers trust to provide a seamless post diagnostic support service.

About you
Good organisation skills are required along with good and non-judgmental communication skills, an understanding of dementia, an understanding of the need for client confidentially and collaborative working and being able to travel independently within the service area.

You will possess NVQ level 3 (or equivalent) or demonstrable experience. It is desirable that you have experience of working with statutory and voluntary agencies.
