About the role

As Dementia Support Worker you will champion the needs of people living with dementia and their carers through raising awareness. You will provide longer term support and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. You will assist people with dementia and their carers to identify their needs and support access to services.  You will recognise that you can’t do it all yourself and that the support of our partner agencies is also important in effectively meeting the needs of people with dementia and their carers, therefore, signposting and onward referrals to other agencies will be essential in your role.  In order to do this you will need to identify and develop links with these agencies in your area.  You will be expected to attend relevant meetings, develop networks and identify and support potential joint working ventures.  You should have good IT skills, be able to travel independently and be able to seize opportunities that are presented to you.
